A support coordinator works closely with you, getting to know your unique needs, goals, and aspirations. They help to put your funding into action, working toward those goals, both short and long term.

Support coordinators make it simple, by identifying suitable providers based on your needs and preferences. They can help arrange appointments, make referrals, and ensure that you have access to the supports that can help you work toward your goals, such as therapy, housing, employment services, and more.  They can even help you to set up Service Agreements with your providers. 

Standing Up For You 

Your support coordinator can help you understand your rights, resolve conflicts, and communicate with service providers, government agencies and other stakeholders. If there comes a time when you need advocacy, they should not act as a formal independent advocate for you, but they can refer you to independent advocacy services.
