Also known as specialist support coordination, complex case management is a type of support for those with complex circumstances. Through our complex case management program, we can help you to achieve your goals and live a happy, fulfilling life. We work closely with you to understand your barriers to receiving supports and develop a tailored complex case management program to suit your individual needs. 

 

What is complex case management and am I eligible?

Complex case management, or specialist support coordination, is a sub-category of NDIS support coordination. Support coordination is a capacity building support that is designed to help connect you with service providers and manage ongoing provider relationships. As defined by the NDIS, specialist support coordination “…is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.”

You may be eligible for NDIS funding for complex case management if you have a barrier that is preventing you from accessing support such as a complicated living situation, difficulty communicating, or living in a household with multiple family members who have a disability. Complex case management is designed to help address these barriers and reduce any complexities that are preventing you from achieving your goals. Whether you live at home with parents or guardians who also have a disability, you suffer from a mental health illness or you’re in supported independent living NDIS, the team at Disability Plan Services have the expertise to help make the most of your plan.  

This type of support is implemented by a specialist NDIS support coordinator. A specialist support coordinator works with you and your family by addressing the complex barriers in your life and provide you with the knowledge and support needed to access your supports.

NDIS support coordination qualifications
