Disability plan reviews – An easy-to-understand guide

It is completely normal for your circumstances to change over time and as a result, your disability supports also need to change. If you’re finding that your current NDIS plan is no longer working for you, it may be beneficial to undergo an early plan review to ensure your goals and needs are being met.

What is a disability plan review? 

After your initial NDIS funding application, your first plan will usually be in place for a period of 12 months, after which time you will have a plan review to discuss how things are going and address any questions or concerns you have.

If you have progressed towards your goals faster or slower than originally anticipated, this is your opportunity to reassess and set new goals if need be. This could result in a change to your NDIS funding or available supports. 

However it is not uncommon to experience a change prior to your scheduled plan review. Sometimes these changes can lead to you requiring an early plan review. Below are just three of the many reasons why you may need to request an early plan review.

  • Your personal circumstances have changed such as your living arrangements
  • Your goals have shifted, either as a result of your progression towards your current plan goals or a change of priorities 
  • You are working with an experienced Plan Manager, such as Disability Plan Services, who believes you may require additional supports 

 

Your plan review can take place either in person or over the phone – whichever works best for you. You are also welcome to bring a support person with you. Having support from someone you know and trust is a great way to ensure you don’t forget to mention anything important. It may also help to have them advocate for you and to make sure you get everything you need from your plan. If you would prefer the support from an experienced Plan Manager, a member from Disability Plan Services can attend your plan review with you.

How to prepare for your plan review

It’s important to be prepared to make sure you’re getting the most of your review. If you no longer feel as though your supports are adequately meeting your needs, the team at Disability Plan Services will take the time to get to know you, understand your specific needs and help you achieve what you want from your plan and NDIS funding application

Here are some key things you should consider before attending your plan review. 

Review your goals 

  • Have I achieved my short-term goals or made progress towards achieving them?
  • Do I need to change any of my goals?

Review your supports

  • What supports do I require to help achieve my goals?
  • Are the disability support services I’m currently receiving helping me achieve my goals?
  • Are there any supports I want to change or not happy with?

Review your funding 

  • Am I happy with how my NDIS funding is being managed?
