Disability Plan Reviews

Your NDIS plan review (disability plan review) is your opportunity to discuss how you are progressing towards your goals and make any changes necessary to your disability support services and funding. The team at Disability Plan Services can help you to prepare for your review, ensuring you are making the most of what is available to you.

It is normal for your needs and goals to change over time, that’s why your plan reviews are an important way to make sure your plan is still working for you and keep you on track towards your long-term goals. Your plan review may seem a little intimidating, so that’s why the Disability Plan Services team is here to support you.

What is a Disability Plan Review?

Your disability plan review (NDIS plan review) is your opportunity to discuss how your plan has been working for you, set new goals and discuss any questions or concerns you have. Your first plan review will usually happen 12 months after your plan started.

Because things change over time, it’s normal for your NDIS plan to change too. You may need less assistance in certain areas but are looking to start a new hobby or join a recreational group which could mean a change in your day-to-day disability support services or transport requirements.

If you’re working with an NDIS Support Coordinator or Plan Manager, such as Disability Plan Services, they will help to prepare you for your plan review by collating reports from your service providers, reflecting on your goals, setting future goals and discussing with you the disability support services you are currently receiving. They can also develop a list of support services for a disability that may be suitable for your next plan. If you and your Disability Plan Services partner feel confident with your progress, you can request for your plan reviews to be up to 3 years apart.

Just like with your NDIS planning meeting, your plan review can happen either in person or over the phone – whichever is best for you. If you choose to conduct your NDIS planning meeting face-to-face, you are welcome to bring a support person with you. Having support from someone you know and trust is a great way to make sure you haven’t forgotten to mention anything. It may help to have them advocate for you and to make sure you’re getting everything you need from your plan. If you’d like, a member of our Support Coordination team can join you at your plan review.

Preparing for your Disability Plan Review

Your plan review is your opportunity to make sure your NDIS plan is still helping you to achieve your goals and make any changes you need. Being properly prepared is the best way to make sure you’re getting the most out of your review.

Here are some things you should consider before your NDIS disability plan review.

What to prepareWhy
What disability support services are you currently receiving that are helping you towards your goals? If you’ve been really happy with something, try to pinpoint what made that part of your plan so successful as this will help you to work towards your long-term goals.
Are there any supports you want to change or you are not happy with?If your circumstances have changed or there are some supports which are not helping you to achieve your goals, discussing this is important to help make the required amendments.
Have you achieved your short-term goals or made progress towards achieving them?Look back at your first NDIS plan and see what progress you have made towards your goals. This is important to make the most out of your plan and live the life you choose.
Do you need to change any of your goals or continue working towards them? This is your opportunity to make changes to your plan including amending any of the short-term goals which you may still be working towards.
What supports do you require to help you achieve your goals? If your current supports are not helping you achieve your goals or if you have made changes to your goals, this is your chance to discuss your options for supports.
Are you happy with how your NDIS funding is being managed? If you are currently self-managing your NDIS funding or your funding is being managed by the NDIA, you can request for additional funds to be allocated for a registered NDIS Plan Manager, such as Disability Plan Services, to assist with all disability plan management.
